How are we supporting?

Patients with Long-Term Conditions

The Practice has developed Nurse-led clinics for management of the following conditions:

  • Asthma
  • Diabetes
  • Hypertension
  • Coronary Heart Disease
  • Hormone Replacement Therapy
  • Weight Management

Once the Doctor is satisfied your condition is stable and you are on the best treatment, he or she will recommend you see the Practice Nurse for regular monitoring of your condition. The Nurses are working under the supervision of the Doctors and to guidelines developed nationally by NICE. The Nurses will arrange any regular blood tests required and ensure you have necessary regular checks. In addition they will aim to improve the control of these conditions by giving you a better understanding of them.

If you do develop any further problems the Nurse will arrange for you to see the Doctor as soon as appropriate. Of course, you are free to see the doctor whenever you have concerns about health and your treatment.

Long-term condition registers

All patients with a long-term condition are included on our registers.  This is to ensure you are included in our recall system to ensure your condition is reviewed regularly and at least once each year.  The quality of care of patients with your condition is audited and discussed within the team.  This is to ensure we are observing the best and recommended practice.

Clinical audit

The Practice has a programme of regular audit and monitoring of issue related to our service.  We are full members of PRIMIS, a service developed by the University of Nottingham, which helps us review our clinical care including long-term conditions and prescribing.

In-house services

As part of our care for patients with long-term conditions, we are able to provide the following services:

  • Lung function testing (spirometry)
  • 24 hour blood pressure monitoring
  • 24 hour ECG

Appropriate referral and investigation

Where your long-term condition requires it, we will arrange for hospital referral for further investigation and treatment.
