A new service is available to Patients from 31st March 2016 in addition to, Booking Appointments, Ordering Repeat Medication and Viewing your Summary Care Record (SCR)
NHS England require GP practices to offer online access to detailed coded information in GP records where requested by the patient from 31st March 2016
Types of coded data available to patients will include:
■ Allergies/Adverse reactions
■ Medication (dose, quantity and last issued date)
■ Results (numerical values and normal range)
■ Codes showing referral made or letters received (no attachments)
Patients will be able to REQUEST to see their detailed coded information in their online GP records.
You would need to apply for Online Access and REQUEST access to your detailed coded records .The level of access will depend on decisions made at the practice, the individual patient and the contents of their record. The vast majority of patients can be given detailed coded record access. A few patients will require more restricted access and on rare occasions no access if it’s in the best interest of the patient.

When viewing your recorded Data, you must take care in determining your interpretation of what the Data is identifying.
If you applied for online access prior to 31/3/2016 for booking appointments and requesting prescription services, you will still need to REQUEST access for viewing detailed coded information. Please ask a member of our reception team for an application form.
Our practice will offers this facility for patients from 1st April 2016 to view online, export or print detailed coded information held in their own records.
