The white slips ‘Patient Feedback’ are for you to give your own thoughts about a member of staff whom you have seen during your visit to the Practice. You may wish to thank them for something in particular or to suggest how their service to you could be improved in the future.
Please remember while feedback is an excellent way for staff to learn and develop, it can also be very upsetting or worse if it is expressed in very negative language or terms. We ask that you keep all feedback polite and well-intentioned.
The ‘Patient Feedback’ slips can be found on the table in the waiting room or a member of staff may ask you to fill one in after your appointment. Please leave them in the blue box provided. Your comments will be passed on confidentially to the member of staff concerned.
